Epidemiologic and etiologic characteristics of infectious diarrhea in Jiangxi Province, 2018-2019

    目的 对2018—2019年江西省感染性腹泻患者病原微生物感染状况和流行特征进行分析,为食源性疾病的防控提供参考。方法 对江西省36家食源性疾病监测哨点医院送检的感染性腹泻患者标本进行病原微生物检测,通过国家食源性疾病监测报告系统收集病例信息及检验结果,并采用描述性流行病学和地理信息系统技术方法进行分析。结果 共采集感染性腹泻患者标本5 817份,检出病原微生物504份,总检出率为8.66%(504/5 817)。其中,沙门氏菌为3.11%(181/5 817),副溶血性弧菌为0.46%(27/5 817),致泻大肠埃希菌为3.87%(225/5 817),志贺氏菌为0.28%(16/5 817),空肠弯曲菌为0.38%(22/5 817),诺如病毒为0.58%(33/5 817)。第三季度的病原微生物检出率最高,各季度间病原微生物总检出率存在显著性差异(P<0.05)。腹泻病例男女比例为1.2∶1,0-19岁组占总病例数的52.14%,0-19岁组检出率(10.55%,320/3 033)与20岁及以上组检出率(6.61%,184/2 784)存在显著性差异(P<0.05)。65岁及以上组检出率(9.88%,56/567)与20-64岁组检出率(5.77%,128/2 217)存在显著性差异(P<0.05)。阳性病例就餐场所主要集中在家庭,可疑暴露食品主要是混合/多种食品、肉与肉制品、水产动物及其制品。南昌( 9.56%)(92/962)、新余(9.33%)(46/493)、景德镇 (9.26%)(55/594)病原微生物检出率较高,赣州(7.22%)(40/554)、吉安(7.02%)(29/413)检出率较低。结论 江西省感染性腹泻病原微生物主要以致泻性大肠埃希菌和沙门氏菌为主,发病人群以未成年人为主,病原微生物检出率呈现赣北、赣中高,赣南低的空间分布特点。

    Abstract:

    Objective To study the epidemiologic and etiologic characteristics of foodborne diarrhea in Jiangxi Province between 2018 and 2019, so as to provide a scientific reference for its prevention and control.Methods Pathogen isolation and identification of foodborne diarrhea samples from the 36 hospitals in Jiangxi Province were carried out. The data collected by National Foodborne Disease Surveillance and Report System were used to analyze the epidemiological characteristics through geographic information system technology and descriptive epidemiological method.Results A total of 5 817 cases of foodborne diarrhea were collected and 504 strains of pathogenic microorganisms were detected, with the overall detection rate at 8.67%(504/5 817). The detection rates of Salmonella, Vibrio parahaemolyticus, diarrhogenic Escherichia coli, Shigella, Campylobacter jejuni and norovirus were 3.11% (181/5 817), 0.46% (27/5 817), 3.87% (225/5 817), 0.28% (16/5 817), 0.38% (22/5 817), 0.58% (33/5 817). The pathogen detection rate was the highest in the third quarter. There was statistically significant difference in the detection rates of the pathogenic microorganisms in different quarters (P<0.05). The male-female ratio was 1.2:1, 0-19 years old group accounted for 52.14% of the total cases. There was statistically significant difference between the detection rates of 0-19 years old group (10.55%, 320 / 3 033) and that of age 20 years and older group(6.61%, 184 / 2 784)(P<0.05). There was statistically significant difference in the detection rates between the group aged 65 and older (9.88%, 56/567) and the group aged 20-64 (5.77%, 128 / 2 217)(P<0.05). The dining places of positive cases were mainly at home. Suspicious food was mainly mixed food followed by meat and meat products and aquatic animals and their products. Nanchang (9.56%) (92/962), Xinyu (9.33%) (46 / 493), Jingdezhen (9.26%) (55 / 594) had higher detection rates of pathogenic microorganisms of foodborne diarrhea, while Ganzhou (7.22%) (40 / 554) and Jian (7.02%) (29/413) had lower detection rates.Conclusion Diarrhogenic Escherichia coli and Salmonella typhimurium were the main pathogens which caused food-borne diarrhea in Jiangxi Province. The majority of patients were juveniles. The detection rates of pathogenic microorganisms showed a geographic characteristics of high in the northern and central Jiangxi, and low in the southern Jiangxi.
