Traceability analysis of a Staphylococcus aureus food poisoning incident based on whole-genome sequencing

    目的 对惠州市博罗县某学校发生一起由金黄色葡萄球菌引起的食物中毒样本进行分子分型及溯源分析,为此类疫情事件的处置和判定提供参考依据。方法 对疑似金黄色葡萄球菌菌株,使用VITEK MS质谱仪进行菌株鉴定,用酶联免疫吸附试验(ELISA)检测肠毒素类型,通过全基因组测序(WGS)分析可疑金黄色葡萄球菌的耐药基因、毒力基因、多位点序列分型(MLST)及全基因组单核苷酸多态性(wgSNPs),基于全基因组序列构建系统发育进化树以追溯病原体来源。结果 共分离鉴定了10株金黄色葡萄球菌分离株,其中包括6株患者肛拭子分离株、3株患者呕吐物分离株和1株食品分离株;肠毒素类型检测结果显示,5株患者肛拭子分离株、3株患者呕吐物和1株食品分离株产A型肠毒素,1株患者肛拭子分离株产B型肠毒素。产A型肠毒素的9株分离株均为ST6型,产B型肠毒素的1株分离株为ST338型。ST6分离株携带多种耐药基因,且均携带肠毒素A基因(sea)和多种溶血素基因;ST338分离株携带肠毒素B基因(seb)。系统发育分析结果表明,ST6型病例分离株与食品分离株属于同一独立亚分支,菌株间SNP突变位点差异为0~14。结论 该食物中毒事件由鸡扒污染的ST6型金黄色葡萄球菌引起,本研究为食源性疾病的防控提供了重要数据支持。

    Abstract:

    Objective To perform molecular typing and traceability analysis on samples from a food poisoning incident caused by Staphylococcus aureus at a school in Boluo County, Huizhou City, and to provide guidance for future management and judgment of similiar epidemic events.Methods Suspected samples were collected, and Staphylococcus aureus was identified using a VITEK MS mass spectrometer. Enzyme-linked immunosorbent assay (ELISA) was used to detect enterotoxin types. Whole-genome sequencing (WGS) was employed to identify resistance and virulence genes, and to perform multilocus sequence typing (MLST) and whole-genome single-nucleotide polymorphism (wgSNP) analysis. A phylogenetic tree was constructed to trace the origin of the pathogen.Results A total of 10 Staphylococcus aureus strains were isolated, including 6 from anal swabs, 3 from vomitus, and 1 from chicken steak. ELISA testing revealed that 8 patient-derived strains and 1 food-derived strain produced type A enterotoxin, while 1 patient-derived strain produced type B enterotoxin. The 9 strains producing type A enterotoxin were all ST6, and the 1 strain producing type B enterotoxin was ST338. The ST6 strains carried multiple resistance genes, and all harbored the enterotoxin A gene (sea) and several hemolysin genes. The ST338 strain carried the enterotoxin B gene (seb). Phylogenetic analysis showed that the ST6 patient-derived strains and the food-derived strain belonged to the same cluster, with SNP differences ranging from 0 to 14.Conclusion This food poisoning incident was caused by ST6 S. aureus contaminating the chicken steak. This study provides important data support for the prevention and control of foodborne diseases.
